Job summary

Horizon Health and Wellness, Inc. is seeking a Care Manager to ensure client caseloads receive coordinated clinical services and advocacy. This role emphasizes compassionate, professional delivery of care across behavioral health services.

You will develop plans based on client needs, maintain teamwork with clinical staff, and manage time effectively in a fast-paced environment. Evening shifts may be required.

Qualifications

• HS diploma or GED plus 4 years in Behavioral Health or combined education/experience to meet Behavioral Health Technician qualifications.
• One year case management experience strongly preferred.
• CPR/First Aid and Crisis De-escalation certification or training to be provided if missing.
• Must be at least 21 years old and possess a valid driver’s license with reliable transportation.
• Experience in behavioral health settings preferred and ability to coordinate with clinical teams.

Responsibilities

• Coordinate client services to ensure timely, coordinated care across the behavioral health continuum.
• Advocate for clients and provide supportive case management and planning.
• Maintain communication with the clinical team and document plans and summaries based on client needs.
• Assist with time management, scheduling and data tracking in a fast-paced environment.
