Centerstone is seeking an intensive case manager to provide wrap-around support, assess needs, and develop individualized care plans for individuals and families. You will work with the service team to identify resources, monitor progress, and ensure compliance with contracts and regulations.

The role involves coordinating with local service agencies, documenting all outreach, and supporting clients through transportation and community resources to achieve service goals.

Qualifikationen

• Bachelor's degree required.
• Master's Degree preferred.
• 4–6 years of experience preferred.

Aufgaben

• Assess treatment needs and develop patient care plans.
• Collaborate with team to determine service needs and level of services.
• Develop and implement the Individual Service Plan.
• Document outreach and ensure services are provided per plan frequency.
• Provide transportation assistance to help participants achieve goals.
