The role involves working with clients in office, hospital, and community settings, developing treatment plans, and coordinating with local partners to ensure continuity of care.

Qualifications

• Bachelor's degree required.
• Good organizational, time management, written and verbal communication skills.
• Ability to develop supportive relationships while maintaining therapeutic boundaries.

Responsabilités

• Provides individual and group mental health and substance use case management.
• Links client and family to needed services and resources.
• Develops, implements, and updates treatment plans with input from client and family.
• Enters documentation into Electronic Medical Record in a timely manner.
• Maintains coordination with local stakeholders and law enforcement as needed.
