A bachelor’s degree in counseling, psychology, social work or related field is required, with a focus on addressing the whole person in various settings—clinic, home, and community.

Qualifications

• Bachelor’s Degree in psychology, social work or related field required.
• Experience with Electronic Health Record preferred.
• Valid driver’s license in good standing required.

Responsibilities

• Help patients navigate the health care system through coaching, education and support.
• Coordinate behavioral and primary care needs and develop strategies to remove barriers to wellness.
• Refer, advocate, and link client and family for additional services as identified.
• Enter written documentation into Electronic Medical Record in a timely manner for all services rendered.
• Connect with clients face to face in the office and community, telehealth, and by phone to address needs.
