Centerstone is seeking a Case Manager for our CT3 program in Alton and Marion, Illinois. This full-time role provides intensive case management, assessment, and wrap-around services to individuals and families, coordinating with the service team to develop and monitor care plans.

Responsibilities include assessing needs, creating service plans, communicating with clients and families, and ensuring compliance with contracts and regulations.

Formación

• Bachelor's degree required.
• Masters degree preferred.
• 4–6 years of experience preferred.

Responsabilidades

• Assess treatment needs and develop patient care plans.
• Collaborate with team to determine service levels and needs.
• Develop and implement the Individual Service Plan with clients.
• Communicate treatment plans to clients and families; monitor progress.
• Provide outreach, advocacy, and follow-up to assigned clients.
• Assist clients in accessing transportation and community resources.
