Job summary

Centerstone is a leading nonprofit behavioral health system offering care across the country. This role provides person-centered support, updates care plans, and coordinates treatment to enhance self-sufficiency for clients with mental health or substance use challenges.

The position serves as primary case holder, maintains clinical charts, and collaborates with the clinical team to ensure timely, accurate documentation and effective treatment planning across settings.

Qualifications

• Bachelor’s level preferred; experience equivalency may be considered in place of education requirement.
• Minimum of two years professional experience working directly with adults or youth and families in behavioral health or related setting preferred.
• CANS/ANSA Certification required.

Responsibilities

• Facilitates use of family, social and community resources and advocates as needed.
• Maintains and updates person-centered care plans and monitors progress.
• Maintains clinical records up to date and ensures timely documentation.
• Acts as liaison with managed care teams and income/insurance coordination.
• Provides individual and group interventions in home, community or office settings.
