PAM Health, LLC is seeking a Case Manager to coordinate care across disciplines and optimize discharge planning for a designated patient population. The role requires reviewing inpatient records to determine appropriate admission and care levels, and collaborating with payors to support financial and quality outcomes.

Ideal candidates will have licensure in the state, current BLS, and 3–5 years of inpatient experience in settings such as acute, IRF, or LTACH.

Formación

• Licensure in the state where the hospital resides is preferred.
• Current BLS certification required.
• Three to five years of inpatient experience preferred, in acute/IRF/LTACH settings.

Responsabilidades

• Coordinate care for designated patient population across disciplines.
• Review inpatient admission records to ensure appropriate level of care.
• Assess discharge needs and coordinate the discharge plan with the team.
• Maintain and enhance payor relationships.
