Qualifications

• Licensure in the state where the hospital resides is preferred.
• Three to five years of inpatient hospital experience is typical.

Responsibilities

• Coordinate care for a designated patient population across inpatient settings.
• Review inpatient admission records to ensure proper utilization of hospital resources and admission decisions.
• Assess discharge needs and coordinate the discharge plan.
• Collaborate with the interdisciplinary team to achieve desired outcomes and maintain payor relationships.
