Job summary

PAM Health Rehabilitation Hospital of Richardson is seeking a Case Manager to coordinate interdisciplinary care for a designated patient population. You will review admission records, assess discharge needs, and coordinate discharge planning to ensure appropriate care levels.

You will work closely with an interdisciplinary team to improve financial and quality outcomes and strengthen payor relationships. Education includes state licensure; preferred disciplines include OT, PT, RN, RRT, SLP, or

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Three to five years of inpatient experience, preferably in an acute, IRF, or LTACH setting.
• Current BLS certification required.

• Review inpatient admission records to ensure appropriate level of care.
