Job summary

PAM Health Rehabilitation Hospital of Venice is seeking a Case Manager to orchestrate interdisciplinary care for designated patient populations. You will review admission records to ensure appropriate utilization and determine the proper level of care, while coordinating discharge planning with the care team.

The role requires licensure in the state and current BLS certification, with 3–5 years of inpatient experience preferred.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Disciplines may include OT, PT, RN, RRT, SLP, or LSW.
• Current BLS certification required.

Responsibilities

• Coordinate care across disciplines for designated patient populations.
• Review inpatient admission records to ensure appropriate utilization and level of care.
• Identify discharge needs and coordinate discharge planning with the team.
• Maintain and enhance payor relationships and support positive financial outcomes.
