Y.A.P.A. Apartment Living Program Inc is seeking a dedicated Case Manager in Nashville, TN. The role involves developing and implementing treatment plan goals in collaboration with a multidisciplinary team and guiding Members through transitions to less structured living arrangements.

Responsibilities include admissions support, incident reporting, group facilitation, and daily living supervision. A driver’s license is required, with emphasis on timely documentation and compassionate Member

Qualifications

• Must have High School Diploma with 3 years' experience in Mental Health, Behavioral Health or Drug/Alcohol field or an Associate's degree with 2+ years in the field.
• Must possess a valid, non-commercial driver's license with a clean driving record and ability to operate company vehicles.
• Excellent communication, interpersonal, organizational, and time management skills.

Responsabilités

• Collaborate with admissions to support the admission and acclimation of new Members into the program setting.
• Complete Incident Reports in accordance with MCO and State processes.
• Collaborate with multidisciplinary team to develop treatment plans for identified Members.
• Complete weekly individual Case Management sessions with Members and document in DAP format.
• Facilitate/co-facilitate all groups and complete documentation.
• Transport Members to appointments as directed by Program Director.
• Observe Member drug and alcohol screenings and document observations.
