Responsibilities include roving residences, coordinating meals, transportation, incident reporting, documentation in the EHR, and facilitating group activities while ensuring safety and compliance with treatment plans. The role emphasizes compassion, confidentiality, and teamwork.

Qualifications

• Must possess a valid, non-commercial driver's license with a clean driving record and ability to operate company vehicles.
• Ability to maintain confidentiality.

Responsibilities

• Provide direct oversight of Member residential living arrangements to ensure safety, cleanliness, and overall environment of recovery.
• Support and document Member self-administration of medications.
• Roving and inspection of residences and Member ADL support as needed.
• Complete Incident Reports and progress notes in the EHR (DAP format) by the end of the shift.
• Facilitate/Co-facilitate groups and coordinate transportation as assigned.
• Support Members with activities of daily living and meal planning; observe and document health and safety practices.
