Job summary

Project Transition in Nashville, TN seeks a full-time Case Manager to support Members with behavioral health needs. Aufgaben include developing treatment goals, crisis management, and coordinating with a multidisciplinary team.

You will document care, provide on-call coverage, and help with daily living activities and community linkages. The role requires strong communication, organizational skills, and a compassionate, trauma-informed approach.

Qualifications

• High School Diploma or Associates degree with relevant healthcare experience.
• 3+ years in mental health, behavioral health or drug/alcohol field preferred.
• Valid driver’s license with clean driving record required.
• Strong communication, organization and confidentiality are essential.

Responsibilities

• Develop and implement treatment goals with the multidisciplinary team.
• Complete incident reports per policy and MCO/state requirements.
• Create treatment plans and document progress in DAP notes weekly.
• Provide on-call support and crisis management as needed.
• Assist with transportation and daily living activities for Members.
• Engage families and coordinate with community services.
