Youth Opportunity & Harpeth Primary Assessment Center in Nashville, TN is hiring a Case Manager to coordinate assessment services, documentation, communication, and transition planning under the clinical supervision of a licensed professional. You will serve as a liaison among youth, families, the Tennessee Department of Children’s Services, the treatment team, and community partners.

Qualifications

• Bachelor’s degree in a human services field.
• Advanced verbal and written communication skills.
• Strong organizational, analytical, documentation, and time-management skills.
• Ability to pass background checks and screenings.

Responsibilities

• Coordinate assessment services and documentation.
• Maintain ongoing communication with youth, families, agency representatives, and treatment team.
• Prepare and review treatment plans, transition plans, and discharge summaries.
• Maintain accurate case records per requirements.
• Serve as a liaison among the program, the Tennessee Department of Children’s Services, outside agencies, and families.
• Collaborate with leadership on performance-improvement activities.
