Qualifications

• Bachelor’s degree preferred in Social Work, Psychology, Counseling, or related field.
• Experience with children/adolescents in foster care or related settings preferred.
• Strong documentation and organizational abilities required.
• Ability to collaborate with DCS, families, and multidisciplinary teams.

Responsibilities

• Manage an assigned caseload of DCS youth and maintain regular contact with each youth.
• Coordinate with DCS Family Service Workers, caregivers, residential staff, schools, therapists, and providers.
• Participate in treatment planning, team meetings, and discharge planning.
• Monitor youths' behavioral, educational, medical, and emotional needs.
• Complete monthly summaries, case notes, assessments, and plan updates on time.
• Monitor appointments, school participation, incidents, and progress toward goals.
• Assist with crisis response, safety planning, placement stabilization, and follow-up.
• Ensure case records comply with DCS requirements and agency policies.
• Advocate for youths' needs while promoting safety and successful transitions.
