Job summary

PCS Mental Health seeks a Case Manager (CM) I–III to join our Nashville-based program. You will help develop treatment plans, coordinate with a multidisciplinary team, and oversee member progress toward goals, including transitions to less structured living settings.

You will conduct weekly CM sessions, document in DAP format, facilitate groups, and provide on-call crisis support. A strong emphasis on compassion, confidentiality, and teamwork guides daily practice.

Qualifications

• High School Diploma with 3 years' experience in mental/behavioral health or drug/alcohol field.
• Associates degree with 2+ years in mental/behavioral health field.
• High School Diploma/Associates with 1 year experience in Project Transition/ PCS Mental Health.

Responsibilities

• Collaborate with admissions to support admission and acclimation of new Members.
• Complete Incident Reports per MCO/state/PCS policy.
• Develop treatment plans with multidisciplinary team.
• Complete weekly individual Case Management sessions with Members and document in DAP format.
• Facilitate/Co-facilitate all groups and document.
• Transport Members to appointments as directed.
• Observe and document member drug/alcohol screenings.
• Phone coaching/crisis management, be available for emergencies when needed.
• Provide coverage for staff callouts.
• Complete incident reports and documentation for on-call emergencies.
• Support Members with community service links.
• Complete family contacts for Members and ensure emergency contacts.
• Ensure all consents are completed and updated yearly.
• Review Advance Directives annually.
• Assist with activities of daily living and self-care tasks.
• Support medication self-administration observation.
• Develop socialization/communication, conflict resolution, and de-escalation skills.
• Assist with meal planning, shopping, and meal prep.
• Encourage health and home safety practices.
• Assist with transportation navigation.
• Complete residential roving and safety checklists.
• Support discharge follow-up.
• Complete continuing education requirements.
• Attend supervision and program meetings.
• Document all Member engagements within 24 hours.
• Other duties as assigned.
