Job summary

Community Bridges, Inc. (CBI) is a nonprofit behavioral health provider in Arizona.

The Case Manager position involves delivering community and in-office case management, coordinating services, and supporting patients and families throughout treatment and recovery. You will arrange access to health services, finances, employment, and social supports while documenting progress and maintaining a recovery-focused environment.

Qualifications

• High school diploma or GED required; associate degree preferred in a related field.
• 1–3 years experience with GMH/SU populations and outreach engagements required.
• Experience in a behavioral health role is preferred; valid AZ driver’s license preferred.
• Arizona Fingerprint Clearance Card may be required by some roles.

Responsibilities

• Provide case management in community or office settings to patients and families.
• Plan, connect, outreach, advocate, coordinate and monitor patient needs and progress.
• Help patients access health services, finances, employment, education, and social supports.
• Assess needs, develop and monitor individualized treatment plans, document services.
