Community Bridges, Inc. (CBI) is a behavioral healthcare provider seeking a Navigator I to support clients by outreach, engagement, and case management. You will connect individuals to needed services and track progress per state and CARF standards.

You will work within a multidisciplinary team, documenting interactions and helping clients overcome barriers toward ongoing success. A valid driver's license and clean driving record are required.

Qualifikationen

• Highschool diploma or GED required; Associates degree in Behavioral Health preferred.
• Minimum six months recovery from substance use and/or mental health disorders preferred.
• 1-3 years of full-time healthcare-related work experience preferred.
• 1-3 years of substance abuse/mental health recovery-oriented service preferred.
• Driver's license in good standing.
• Clear 39-month motor vehicle record.

Aufgaben

• Perform outreach, engagement, case management and wraparound services.
• Document and track client interactions per AAC/ADHS-DBHS/CARF and internal policies.
• Map next steps to access services and support ongoing client success.
