Job summary

Community Bridges, Inc. (CBI) is seeking a Case Manager to provide community and office-based case management, coordinating services and advocating for clients and families. The role emphasizes recovery, engagement, and access to health, housing, employment, social supports.

Required: 1–3 years in behavioral health, high school diploma, AZ driver license preferred; offers a strong benefits package and opportunities for growth.

Qualifications

• Highschool diploma or GED required. Associates degree of higher in a field related to behavioral health preferred.
• 1-3 years of experience working with the GMH/SU population required.
• 1-3 years of experience in behavioral health position providing outreach and engagement activities required.
• 1-3 years of experience in a medical setting preferred.
• Minimum of 6 months of recovery from substance use and/or mental health disorders preferred.
• Arizona Fingerprint Clearance Card.

Responsibilities

• The Case Manager provides case management services in either the community and/or in office setting to patients and their families.
• The Case Manager assists the patient and care team in planning, connecting, outreaching, advocating, coordinating and monitoring the patient’s needs and progress.
• The Case Manager assists patients in gaining access to health services, financial assistance, employment, education, social services and natural supports.
• The Case Manager assesses patient needs, develops and monitors individualized treatment plans and documents services provided in an accurate and timely manner.
• Embraces the recovery philosophy and promotes a safe recovery environment for clients.
