Job summary

Community Bridges, Inc. (CBI) is seeking a Case Manager to provide case management services in community and office settings, coordinating and monitoring patient needs and progress.

The role requires 1–3 years in GMH/SU populations with outreach/engagement experience; a high school diploma or GED is required, with associate degree preferred. Arizona Driver’s license and fingerprint clearance are favorable.

Qualifications

• High school diploma or GED required; Associate degree or higher in behavioral health preferred.
• 1–3 years experience with GMH/SU population required.
• 1–3 years experience in behavioral health with outreach/engagement required.
• 1–3 years experience in a medical setting preferred.
• Arizona Fingerprint Clearance Card preferred.

Responsibilities

• Provide case management services in community and/or office settings.
• Assist patients and care team in planning, connecting, outreaching, advocating and coordinating.
• Assess patient needs and develop/monitor individualized treatment plans.
• Document services accurately and timely; promote recovery environment.

Education
