Job summary
Community Bridges, Inc. (CBI) is seeking a dedicated Case Manager to support individuals with serious mental illness in the community.
You will assist with planning, advocacy, coordination and monitoring to help clients access health services, employment, education, and supports, while documenting services accurately and timely. The role emphasizes recovery-centered care, safety, and collaboration with families and healthcare teams.
Qualifications
- High school diploma or GED required; associates or higher preferred.
- Minimum of 1 year in a behavioral health position.
- Six months or more recovery from substance use or mental illness preferred.
- Valid AZ driver's license and clean MVR are required.
- BHT certification and fingerprint clearance per policy.
Responsibilities
- Provide community-based case management to patients with serious mental illness and families.
- Assist in planning, advocacy, coordination and monitoring of services to access health and social supports.
- Assess needs, develop and monitor individualized treatment plans and document services.

CBI is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting-edge, nationally recognized, evidence-based models.
The SMI Case Manager Provides case management services in the community to patients with serious mental illness and their families. Assists in planning connecting, advocacy, coordination and monitoring; to assist patients in gaining access to health services, financial assistance, employment, education, social services and natural supports. Assesses needs, develops and monitors individualized treatment plans and documents services provided in an accurate and timely manner. Embraces the recovery philosophy and promotes a safe recovery environment for clients.
- Highschool diploma or GED required. Associates degree or higher in a field related to behavioral health preferred.
- 1 year of experience in a behavioral health position.
- Minimum of six months of recovery from substance use and/or mental health disorders preferred.
- Current AZ Driver's License (valid and in good standing)
- Clear 39-month Motor Vehicle Record (MVR)
- Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures
- Peer Certification (if applicable)
- Arizona Fingerprint Clearance card required
