Job summary

Applegate Recovery Services is seeking a dedicated Case Manager to support therapists and enhance the treatment process in our outpatient clinic. You will liaison between therapists, families, and referral sources, arrange visits and transportation, and prepare reports for treatment staffing.

Experience with adults with addiction and a CDCA are preferred; strong organizational and communication skills are essential.

Qualifications

Experience working with adults with addiction is preferred.
Must have a CDCA.
Strong written and verbal communication with families and providers.
Excellent organizational and multitasking abilities.
Ability to prepare clear treatment-related reports.

Responsibilities

Act as liaison between therapist, referring agency, and family; arrange visits and transportation.
Ensure ongoing communication between family, referral source and Mahajan therapeutics.
Attend court hearings as appropriate.
Attend treatment staffing with prepared reports.
Facilitate individual and group CSP as assigned.
Attend supervisory sessions with prepared agenda.
Assist clients in achieving personal independence in daily living skills.
Support development of daily living skills as identified.
