Role Description

This is a full-time, on-site Case Manager role based in Huntington Beach, CA. The Case Manager will coordinate client care plans, conduct intake assessments, and maintain accurate, confidential documentation of client progress. Responsibilities include collaborating with therapists and other treatment providers, connecting clients with appropriate resources and support services, and monitoring adherence to treatment recommendations. The Case Manager will also communicate regularly with clients and, when appropriate, families or support networks to ensure continuity of care and address emerging needs. In addition, the role involves participating in team meetings, supporting crisis intervention efforts, and contributing to a safe, respectful, and trauma-informed environment.

• Knowledge of addiction, mental health, and trauma-informed practices, with the ability to apply these in daily client interactions.
• Organizational and documentation skills, including accurate record-keeping and adherence to confidentiality and compliance standards.
• Ability to handle sensitive situations with empathy, maintain professional boundaries, and support crisis intervention when needed.
• Experience in a behavioral health, substance use treatment, or social services setting is preferred.
• Relevant licensure or certification (e.g., CADC, LCSW, LMFT, or equivalent) and/or a degree in psychology, social work, counseling, or related field is highly beneficial.
• Availability to work part-time on-site in Huntington Beach, CA, and willingness to collaborate in a team-oriented environment.
