Job summary

Woodlake Addiction Recovery Center is seeking a highly motivated Case Manager to join our team. We are dedicated to providing clients with compassionate support and evidence-based care as they navigate recovery from addiction.

Responsibilities include conducting assessments, developing treatment plans, coordinating with clinical staff, referrals, progress monitoring, and maintaining confidential records. A Bachelor's or Master's in social work or related field and at least two years of case

Qualifications

• Bachelor's or Master's degree in social work, psychology, counseling, or related field.
• Minimum of 2 years of experience in case management or related field.
• Knowledge of addictions, mental health, and trauma-informed care.
• Strong communication and interpersonal skills.
• Ability to work collaboratively as part of a team.
• Experience with electronic medical records.

Responsibilities

• Conduct comprehensive assessments of clients' needs and develop individualized treatment plans.
• Collaborate with clinical staff to coordinate treatment and ensure the implementation of the treatment plan.
• Provide case management services, including referrals to external resources and follow-up appointments.
• Monitor client progress and provide guidance and support throughout their recovery journey.
• Communicate and collaborate with clients' families and loved ones to ensure a holistic approach to treatment.
• Maintain accurate and confidential records of clients' progress and treatment plans.
• Participate in team meetings and provide updates on clients' progress and any challenges or concerns.
• Keep up-to-date with industry trends and best practices to continuously improve the quality of care provided.
