Job summary

DeCoach Recovery Centre is seeking a Case Manager to provide community-based case management for adults with substance use and mental health disorders, using a company vehicle. You will connect clients to resources, advocate for services, and help build daily living skills.

You will deliver individualized care per each client’s treatment plan, coordinate with the treatment team, and review services with supervisors to identify improvements. Documentation and policy compliance are essential.

Qualifications

• High school diploma or equivalent is required or welcomed college degree.
• At least 1 year of experience in behavioral health or a related field (from the job description).
• Active Ohio driver's license with a clean driving record for the past 3 years.

Responsibilities

• Meet clients where they are: provide mostly community-based case management to adults with substance use and mental health disorders, using a company vehicle.
• Connect clients to resources: link clients with services, advocate on their behalf, and help them build daily living skills.
• Deliver individualized care: provide services outlined in each client’s Individual Treatment Plan, including case management, activities, and coordination of care.
• Work as part of a team: collaborate with the Treatment Team to ensure plans fit the clients’ needs.
• Help us get better: review services with admins and team managers and suggest improvements.
• Keep it documented: follow documentation procedures and stay current on DeCoach policies.

What You’ll Do

• Meet clients where they are: Provide mostly community-based case management to adults with substance use and mental health disorders, using a company vehicle
• Connect clients to resources: Link clients with services, advocate on their behalf, and help them build daily living skills
• Deliver individualized care: Provide the services outlined in each client’s Individual Treatment Plan, including individual and group case management, community activities, facility visits, vocational training, and coordination of care
• Work as part of a team: Collaborate with the Treatment Team to make sure each client’s plan fits their needs
• Help us get better: Partner with administrative and team managers to review our services, assess needs, and recommend improvements
• Keep it documented: Follow correct documentation procedures and stay current on DeCoach policies and procedures

Flexibility: This role may use flex hours so you can serve clients when they need you most.

What You Bring

• High school diploma or equivalent (a college degree from an accredited institution is welcome)
• At least 1 year of experience in behavioral health or a related field
• Working knowledge of behavioral health issues and current therapeutic methods
• An active Ohio driver’s license with a clean driving record for the past 3 years
