Job summary

DeCoach Recovery Centre in Fairfield, OH is seeking a Case Manager to provide community-based case management to adults with behavioral health needs. You’ll connect clients to resources, advocate on their behalf, and help them build daily living skills.

You will deliver care per each client’s individualized plan, collaborate with the treatment team, and support documentation and policy compliance. This is a full-time role with opportunities to impact recovery outcomes.

Qualifications

• High school diploma or equivalent required; college degree welcomed.
• At least 1 year of experience in behavioral health or related field.
• Working knowledge of behavioral health issues and current methods.
• Ohio driver's license with a clean record in the last 3 years.

Responsibilities

• Meet clients where they are, providing community-based case management using a company vehicle.
• Connect clients to resources, advocate for services, and help build daily living skills.
• Deliver services per each client’s Individual Treatment Plan, including group and individual activities.
• Collaborate with the Treatment Team to ensure plans fit client needs.
• Review services with admins and team managers to identify improvements.
• Maintain proper documentation and adhere to DeCoach policies.

As our Case Manager, you’ll meet clients where they are, both literally and figuratively, connecting them with the resources, skills, and support they need to reach their personal recovery goals. You’ll work under clinical supervision as part of a collaborative treatment team, and you’ll get to see the difference you make.

What You’ll Do

• Meet clients where they are: Provide mostly community-based case management to adults with substance use and mental health disorders, using a company vehicle
• Connect clients to resources: Link clients with services, advocate on their behalf, and help them build daily living skills
• Deliver individualized care: Provide the services outlined in each client’s Individual Treatment Plan, including individual and group case management, community activities, facility visits, vocational training, and coordination of care
• Work as part of a team: Collaborate with the Treatment Team to make sure each client’s plan fits their needs
• Help us get better: Partner with administrative and team managers to review our services, assess needs, and recommend improvements
• Keep it documented: Follow correct documentation procedures and stay current on DeCoach policies and procedures

Flexibility: This role may use flex hours so you can serve clients when they need you most.

What You Bring

• High school diploma or equivalent (a college degree from an accredited institution is welcome)
• At least 1 year of experience in behavioral health or a related field
• Working knowledge of behavioral health issues and current therapeutic methods
• An active Ohio driver’s license with a clean driving record for the past 3 years
