Job summary

Community Care, Inc. is seeking a Case Manager/Care Manager to join our Family Care Program team in Outagamie County, WI. The role focuses on community-based case management, comprehensive assessments, and coordinated care planning to help seniors and adults with disabilities live independently.

The position is full-time, Monday–Friday, 8:00 AM to 4:30 PM, with regular local travel. A $5,000 sign-on bonus is offered to new hires who meet criteria, reflecting our mission-driven, collaborative

Qualifications

Bachelor's Degree in a Human Services or closely related area required.
A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may substitute for a non-human services degree.
One year of experience with older adults or persons with disabilities in a social service capacity preferred.
Eligibility for long-term care functional screener certification is required.
Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Responsibilities

Manages an assigned caseload of program members in collaboration with an RN Care Manager.
Completes comprehensive member assessments, member centered care plans, & functional screens.
Provides ongoing care coordination and quality assurance monitoring of services.
Maintains required documentation in adherence to established standards.
