Job summary

Community Care is seeking a Program Supervisor for our Family Care Program in Racine, WI. The role leads a Case Management team, oversees daily operations, and partners with senior leaders to ensure high-quality services for older adults and people with IDD, with occasional travel to other offices and community events.

Qualified applicants will have a Bachelor's degree in a relevant field, plus 3 years of case management experience and leadership experience.

Qualifications

• Bachelor's degree in Nursing, Psychology, Social Work or Human Services required.
• Long-term care functional screener certification preferred (eligibility for this certification is required). RN preferred.
• Minimum of 3 years experience providing case management services for older adults, IDD; leadership including direct supervision preferred.

Responsibilities

• Provide leadership and mentoring for our Case Management team.
• Directly engage in Case Management Activities.
• Coordinate and supervise the day-to-day operations of the Family Care Program in collaboration with other leaders.
• Routinely travel locally, use of personal vehicle, valid driver's license, and proof of required insurance coverage necessary.

Community Care is hiring a Program Supervisor for our Family Care Program. This is a full-time opportunity. The position is based in the Community Care Racine office but requires at times travel to other CCI offices, events in the community and member visits.

Program Supervisor Job Responsibilities:

Program Supervisor Job Requirements:

Education: Bachelor's Degree in Nursing, Psychology, Social Work or Human Services required. Licensure: Current long-term care functional screener certification preferred (eligibility for this certification is required). RN preferred.

A minimum of 3 years experience providing case management services for older adults, intellectual or developmentally disabled (IDD). Leadership experience including direct supervision of staff preferred.

Program Supervisor Job Schedule:

Monday to Friday - 8:00 AM – 4:30 PM
