Job summary

Community Care, Inc. is seeking a Registered Nurse (RN) for day shift in Milwaukee, WI. This role includes a $10,000 sign-on bonus and a Monday–Friday schedule from 8:00 am to 4:30 pm.

Join a mission-driven team serving older adults and individuals with disabilities in the community, with a focus on collaborative care and patient-centered planning. You will manage caseloads, conduct comprehensive assessments, develop care plans, coordinate services, educate members, and participate in quality

Qualifications

• Must have an active Wisconsin RN license.
• Associate's degree required; bachelor's preferred.
• Experience with older adults or disabilities is valued.

Responsibilities

• Manage caseload of Family Care Program members.
• Complete comprehensive clinical assessments.
• Develop member-centered care plans and coordinate ongoing care.
• Provide member education, quality assurance, and monitoring.
• Travel locally using personal vehicle with valid license.

Registered Nurse (RN) Job Responsibilities:

• Manage caseload of Family Care Program members
• Complete comprehensive clinical assessments
• Working collaboratively to develop, member centered care plans, provide on-going care coordination, member education, quality assurance and monitoring of services.
• Travel, Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Registered Nurse (RN) Job Requirements:

• Education: Associate's Degree in Nursing Required; Bachelor's degree in Human Services or closely related field; preferred.
• Licensure: Wisconsin Registered Nurse/RN license required. Current long-term care functional screener certification preferred (eligibility for this certification is required)
• Experience: Minimum of 3 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity required. 3 years of work in community nursing preferred.

RN Nurse Work Hours - Monday through Friday position, 8:00 am to 4:30 pm.
