Responsibilities

  • Conduct client assessments and develop individualized wellness plans.
  • Provide short- and longer-term case management based on client needs.
  • Help clients address financial emergencies and develop strategies for greater financial stability.
  • Assist clients with budgeting, prioritizing expenses, reducing barriers, and developing strategies for stability.
  • Connect clients with public benefits and community resources related to housing, food, employment, transportation, healthcare, and other needs.
  • Assist clients with benefit applications and online systems when appropriate.
  • Advocate for clients and help them navigate complex service systems.
  • Provide trauma-informed, respectful services that promote dignity and self-determination.
  • Monitor client progress, provide follow-up, and support transitions toward self-sustainability.
  • Maintain confidential client records and required program documentation.
  • Collaborate with Neighborhood House staff and community partners.
  • Participate in community outreach, resource fairs, meetings, and events.

JOB TITLE: Case Manager

JOB STATUS: This is a full-time position at 35-40 hours per week. The schedule is Monday through Friday, 8:00/9:00 a.m. - 4:00 p.m. May require some evening meetings or outreach for clients who work full time.

REPORTS TO: Director of Client Services

JOB DESCRIPTION:

The Case Manager provides compassionate, client-centered case management services to clients’ experiencing financial hardship, housing instability, food insecurity, transportation barriers, and other life circumstances that may threaten their financial and overall well-being.

The focus of the role is enabling the client to move toward self-sustainability. The Case Manager will analyze the client's budget, provide positive financial coaching, navigate through various financial assistance tools through discovery of their spending behavior, and design a target-based wellness plan that will support the client's journey to self-sustainability.

COMPENSATION: $21 per hour

JOB DUTIES:

  • Conduct client assessments and develop individualized wellness plans.

  • Participate in community outreach, resource fairs, meetings, and events.

JOB SKILLS & REQUIREMENTS:

Required:

  • Experience in case management, social services, human services, client advocacy, or a related field.
  • Strong communication, listening, organization, and problem-solving skills.
  • Strong budget analysis, creation, and balancing skills.
  • Ability to build trusting professional relationships while maintaining confidentiality and boundaries.
  • Ability to manage multiple clients, priorities, and deadlines.
  • Computer proficiency and ability to learn client databases and online benefit systems.
  • Ability to work independently and as part of a team.
  • Valid driver’s license and reliable transportation if required for outreach or client-related activities.

Preferred:

  • Experience working with individuals or families experiencing poverty or financial crisis.
  • Knowledge of Oakland County and Southeast Michigan community resources.
  • Experience with benefits navigation, housing stabilization, utility assistance, food insecurity, transportation assistance, budgeting, or financial coaching.
  • Knowledge of trauma-informed care.
  • Nonprofit or community-based human services experience.

JOB QUALIFICATIONS:

  • Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Counseling, Public Administration, or a related field;
  • Will consider 4+ years of experience in human services, nursing, budget counselling or equivalent life experience demonstrating success in the socioeconomic topics listed above.
