Job summary

Arbor-Circl in Grand Rapids, MI is seeking a CASE MANAGER for Youth Supportive Housing. The role provides case management and direct support to program participants, promoting independent living skills, budgeting, and family reunification in collaboration with hosts and families.

The position requires a Bachelor’s degree in Social Work or related field, experience with at-risk youth, a valid driver’s license, and flexibility for occasional evenings to meet client needs.

Qualifications

• Bachelor’s degree in Social Work or a related field.
• Demonstrated experience working with older adolescents and persons experiencing or at risk of being unhoused.
• Comfortable and willing to meet clients where they are at in the community.
• Valid driver’s license, clean driving record and current auto insurance requiredd
• Ability / availability to work occasional evenings to accommodate client needs.

Responsibilities

• Provides case management and direct support to program participants to achieve project outcomes, including independent living skills coaching, budgeting support, goal planning, resource coordination, and facilitating family reunification sessions with youth and their parent(s)/guardian(s).
• Collaborates with the Kent County Youth Action Board on recruitment, service delivery strategies, and host engagement and retention.
• Maintains weekly communication with youth, advocates for and represents youth interests in program meetings and disputes, communicates youth needs and preferences to hosts, families, and program staff, and supports youth participation in family reunification efforts.

Salary Range: $37,500.00 To $55,000.00 Annually
