Job summary

LHH is seeking a compassionate Case Manager to support individuals experiencing homelessness in achieving housing stability and self-sufficiency. The role is onsite in San Jose, CA with a Sun–Thu schedule and a pay range of $26–$31 per hour.

Responsibilities include conducting intakes and assessments, connecting clients to housing, healthcare, and community resources, and providing ongoing case management, advocacy, and crisis intervention.

Qualifications

  • A minimum of 1 year of experience in case management, homeless services, healthcare, behavioral health, social services, or related field preferred.
  • Passion for serving vulnerable populations and supporting housing stability.
  • Valid driver's license, reliable transportation, and auto insurance.
  • Ability to pass background check, drug screen, and TB test.
  • CPR/First Aid certification or willingness to obtain.
