Job summary

Southwest Key Programs in Dallas, TX seeks a Case Manager to coordinate service plans, assessments and case files within a web-based program, with documentation maintained in both digital and physical files.

The role requires a Bachelor's degree in a social services field and at least two years of experience working with at-risk youth and families. You will conduct intake, provide crisis support, and travel to program sites as needed.

Qualifications

  • Education: Bachelor's in a social service or related field and 2 years’ experience with youth at risk and families.
  • Must read, write, and communicate clearly with internal and external stakeholders.
  • Requires computer proficiency and ability to operate standard office equipment.
  • Background check from appropriate entity required.
  • Driver's license and reliable transportation with vehicle insurance required.

Responsibilities

  • Conduct initial intake with client and family/guardian.
  • Ensure timely completion of intake paperwork, assessments and ISP.
  • Maintain daily documentation in SWK database.
  • Maintain physical and web-based client files.
  • Establish and maintain therapeutic relationships with clients and families.
  • Facilitate and/or co-facilitate group counseling and recreation sessions.
  • Provide crisis intervention for clients and families.
  • Provide weekly home visits with thorough documentation in SWK database.
  • Ensure clients participate in all program components and services.
  • Provide advocacy per SWK guidelines and funder requirements.
  • Attend required trainings and workshops.
  • Communicate openly with Program Director, clinicians and team members.
  • Transport clients to activities, court appointments, school hearings or meetings.

Position Summary:TheCaseManagerisresponsibleforthesubmissionofserviceplansandotherassessmentsrequiredforthecompilingofacomprehensivecasefilefoundonSouthwestKeyProgramsdesignatedweb-basedprogramwithsupportingdocumentationmaintainedinphysicalfile.TheCaseManagermaintainsaflexible,organizedandefficientworkscheduleandissubjecttoworkmorethananormalworkdayandbeoncalltofacilitateprogramservices.

Essential Functions
  • Conduct initial intake with client and family/guardian.
  • Ensure the timely completion of all Intake paperwork, Assessments and Individual Service Plans in accordance with Southwest Key Programs (SWK) policy.
  • Ensure all daily documentation is completed and entered SWK database in a timely and efficient manner.
  • Maintain physical and web-based client files.
  • Establish and maintain therapeutic relationships with clients and families.
  • Facilitate and/or co-facilitate group counseling and recreation sessions.
  • Provide crisis intervention for clients and their families.
  • Provide weekly home visits with clients and families completing thorough documentation in SWK database system.
  • Ensure clients participate or are offered all components or services offered at the program.
  • Provide advocacy specified by Southwest Key Programs and contract guidelines.
  • Attend and participate in all required professional training and workshops.
  • Maintain open and consistent communication with Program Director or designee, clinician and other team members.
  • Transport clients to program activities, court appointments, school hearings or other treatment meetings as needed.
  • Able to react to change productively and handle other essential tasks as assigned.
  • Must be prepared to administer emergency First Aid, CPR, AED and/or Handle with Care techniques to youth and adults regardless of the time of day or hour of the shift, the size of the youth, or the staff's level of personal fatigue.
  • Operate a motor vehicle on public roads as required by the position.
  • Perform additional responsibilities and duties as assigned, consistent with the scope and level of the position.
Qualifications (Education, Experience, and Requirements)
  • Education Requirements: Bachelor's degree in a social service or related field and (2) years’ experience working with youth at risk youth and their families; may include part time, volunteer or internship experience or a master’s degree in social services or related field.
  • Ability to clearly read, write, and communicate information to internal and external stakeholders.
  • Requires computer proficiency and the ability to effectively operate standard office equipment.
  • Works well independently and as an effective team member.
  • Clear background check from appropriate entity.
  • Obtain and maintain First Aid, CPR, AED certification and/or Handle with Care certification as required by funder.
  • Maintain a valid driver's license and status as a qualified driver under the Company’s Vehicle Safety Guidebook standards.
  • Must maintain reliable transportation with required registration and vehicle insurance as required by funder.
Work Environment and Physical Requirements

This position involves daily interaction at various client locations, with work schedules subject to change based on operational needs. The work environment may include noise levels ranging from mild to moderate. Essential functions of the position require extended periods of driving, exposure to varying weather conditions, and the physical ability to bend, stoop, climb, and lift up to 20 pounds, with or without reasonable accommodation.
