Qualifications

  • 2 years’ experience working with youth at risk and their families.
  • Master's degree is acceptable in lieu of part of experience.
  • First Aid, CPR and AED certifications required or pursued.
  • Handle with Care certification may be required by funder.
  • Background check must be clear.

Responsibilities

  • Establish therapeutic relationships with clients and families.
  • Provide group counseling and recreation sessions as needed.
  • Perform crisis intervention for clients and families.
  • Complete weekly home visits with thorough documentation.
  • Transport clients to activities, court appointments, or meetings.
  • Participate in required trainings and workshops.
  • Maintain communication with supervisor and team members.
  • Ensure service components offered at the program are utilized.

Salary Range: $24.03 - $24.03 Hourly

Position Summary

The Case Manager is responsible for the submission of service plans and other assessments required for the compiling of a comprehensive case file found on Southwest Key Programs designated web-based program with supporting documentation maintained in physical file. The Case Manager maintains a flexible, organized and efficient work schedule and is subject to work more than a normal workday and be on call to facilitate program services.

Essential Functions
  • Conduct initial intake with client and family/guardian.
  • Ensure the timely completion of all Intake paperwork, Assessments and Individual Service Plans in accordance with Southwest Key Programs (SWK) policy.
  • Ensure all daily documentation is completed and entered SWK database in a timely and efficient manner.
  • Maintain physical and web-based client files.
  • Establish and maintain therapeutic relationships with clients and families.
  • Facilitate and/or co-facilitate group counseling and recreation sessions.
  • Provide crisis intervention for clients and their families.
  • Provide weekly home visits with clients and families completing thorough documentation in SWK database system.
  • Ensure clients participate or are offered all components or services offered at the program.
  • Provide advocacy specified by Southwest Key Programs and contract guidelines.
  • Attend and participate in all required professional training and workshops.
  • Maintain open and consistent communication with Program Director or designee, clinician and other team members.
  • Transport clients to program activities, court appointments, school hearings or other treatment meetings as needed.
  • Able to react to change productively and handle other essential tasks as assigned.
  • Must be prepared to administer emergency First Aid, CPR, AED and/or Handle with Care techniques to youth and adults regardless of the time of day or hour of the shift, the size of the youth, or the staff's level of personal fatigue.
  • Operate a motor vehicle on public roads as required by the position.
Qualifications
  • Education Requirements: Bachelor's degree in a social service or related field and (2) years’ experience working with youth at risk youth and their families; may include part time, volunteer or internship experience or a master’s degree in social services or related field.
  • Ability to clearly read, write, and communicate information to internal and external stakeholders.
  • Requires computer proficiency and the ability to effectively operate standard office equipment.
  • Works well independently and as an effective team member.
  • Clear background check from appropriate entity.
  • Obtain and maintain First Aid, CPR, AED certification and/or Handle with Care certification as required by funder.
  • Maintain a valid driver's license and status as a qualified driver under the Company’s Vehicle Safety Guidebook standards.
  • Must maintain reliable transportation with required registration and vehicle insurance as required by funder.
Work Environment and Physical Requirements

This position involves daily interaction at various client locations, with work schedules subject to change based on operational needs. The work environment may include noise levels ranging from mild to moderate. Essential functions of the position require extended periods of driving, exposure to varying weather conditions, and the physical ability to bend, stoop, climb, and lift up to 20 pounds, with or without reasonable accommodation.
