Qualifications

• Associate's Degree (AA) in a related field of study with 1–3 years relevant experience.
• Bachelor's degree (BA/BS) preferred with 3–5 years relevant experience.
• Excellent written and verbal communication skills required.

Responsibilities

• Conduct comprehensive client assessments to determine needs and service eligibility; develop and monitor individualized service plans.
• Provide direct support and guidance, including connecting clients to housing, employment training, financial aid, and counseling.
• Maintain accurate documentation of client interactions and progress; advocate within the community and navigate social services.
• Collaborate with multidisciplinary teams to coordinate services and engage in outreach to raise program awareness.
• Engage in ongoing learning and performance improvement to contribute to team and organizational success.

Role Overview

The Case Manager - Empowering Families conducts comprehensive client assessments, develop and coordinate individualized service plans, provide advocacy and resource support, maintain accurate documentation, collaborate with care teams, and promote continuous learning to improve client outcomes.

Job Responsibilities

• Conduct comprehensive assessments to determine client needs and service eligibility. Develop, implement, and monitor individualized service plans in collaboration with clients.
• Provide direct support and guidance to clients, offering emotional support and practical resources. Facilitate access to services, including housing assistance, employment training, financial aid, and counseling.
• Maintain accurate and timely documentation of client interactions, progress, and outcomes in compliance with CCAC policies. Advocate for clients within the community and assist in navigating social services and resources.
• Collaborate with multidisciplinary teams to coordinate services and ensure comprehensive care. Engage in outreach efforts to raise awareness of available programs and services.
• Engage in ongoing learning, training, and skill development, seeking and applying feedback to continuously improve performance and contribute to team and organizational success.

Education and Experience Requirements:
