Job summary

Catholic Charities of the Archdiocese of Chicago is seeking a Case Manager - Resource Hub to assess client needs, develop service plans, connect individuals to resources, and advocate for positive outcomes within the community.

You will collaborate with partners, maintain compliant documentation, and engage in ongoing skill development to support families and communities in Cook and Lake counties.

Qualifications

• Associate's Degree (AA) in a related field; 1-3 years relevant experience.
• Proficiency in Microsoft Office suite.
• Excellent written and verbal communication skills.
• Strong interpersonal skills.
• Ability to work effectively both independently and as part of a team.

Responsibilities

• Conduct comprehensive assessments to determine client needs and service eligibility. Develop, implement, and monitor individualized service plans in collaboration with clients.
• Provide direct support and guidance to clients, offering emotional support and practical resources. Facilitate access to services, including housing assistance, employment training, financial aid, and counseling.
• Maintain accurate and timely documentation of client interactions, progress, and outcomes in compliance with CCAC policies and external requirements of regulatory bodies. Advocate for clients within the community and assist in navigating social services and resources.
• Collaborate with multidisciplinary teams to coordinate services and ensure comprehensive care. Engage in outreach efforts to raise awareness of available programs and services.
• Engage in ongoing learning, training, and skill development, seeking and applying feedback to continuously improve performance and contribute to team and organizational success.

Role Overview

Job Responsibilities

• Kneel and move from sitting, bending, kneeling, or standing multiple times a day

Minimum

• Associate's Degree (AA) in a related field of study 1-3 years relevant experience
• Proficiency in Microsoft Office suite

Preferred

• Bachelor's Degree in Arts/Sciences (BA/BS) in a related field of study 3-5 years relevant experience
• Experience working in a social service environment
• Crisis intervention and de-escalation skills.
