Qualifications

  • Bachelor's Degree in Arts/Sciences with 1-3 years relevant experience.

Responsibilities

  • Assess prospective clients for program eligibility and conduct housing inspections.
  • Develop, implement, and monitor individualized service plans and connect clients to resources.
  • Document client interactions, progress, and outcomes per CCAC policies and regulatory requirements.
  • Collaborate with multidisciplinary teams and community partners to coordinate services.
  • Provide emotional support and guidance to clients.

Role Overview

The Case Manager - Housing Stability d etermines client eligibility, conducts assessments and housing inspections, develops service plans, connects clients to resources, maintains accurate documentation, advocates for clients, collaborates with partners, and supports clients in achieving stability and self-sufficiency.

Job Responsibilities
  • Assess prospective clients for program eligibility through receipt of external referrals. Responsible for collecting and reviewing all documentation to determine program eligibility. Responsible for apartment inspections to ensure it is safe and meets habitability standards. Conduct comprehensive assessments to determine client needs and service eligibility. Develop, implement, and monitor individualized service plans in collaboration with clients.
  • Provide direct support and guidance to clients, offering emotional support and practical resources. Facilitate access to services, including housing assistance, employment training, mainstream benefits, financial aid and counseling.
  • Maintain accurate and timely documentation of client interactions, progress, and outcomes in compliance with CCAC policies and external requirements of regulatory bodies. Advocate for clients within the community and assist in navigating social services and resources.
  • Collaborate with multidisciplinary teams to coordinate services and ensure comprehensive care. Engage in outreach efforts to raise awareness of available programs and services.
  • Engage in ongoing learning, training, and skill development, seeking and applying feedback to continuously improve performance and contribute to team and organizational success.

PLEASE NOTE: Essential functions include all other duties and responsibilities as assigned.

Education and Experience Requirements:
  • Bachelor's Degree in Arts/Sciences (BA/BS) 1-3 years relevant experience

  • Proficiency in Microsoft Office suite
Preferred
  • Master's Degree (MA/MS) 3-5 years relevant experience
  • Prior case management experience
  • Spanish Profiecency (written and spoken)
Certifications and Licensure
  • Preferred LSW or LCSW

$25.00 - $28.00 / hour
