Job summary

Catholic Charities of the Archdiocese of Chicago seeks a Case Manager - Housing Stability to determine eligibility, conduct assessments, develop service plans, connect clients to resources, and advocate for stability.

You will collaborate with partners, maintain documentation, and support clients in achieving independence, coordinating services, and navigating social services. This is a full-time position with comprehensive benefits, offering a wage range of $25.00–$28.00 per hour.

Qualifications

  • Bachelor's degree required with 1–3 years relevant experience; excellent written and verbal communication skills.
  • Strong interpersonal skills and ability to work independently and in a team.
  • Proficiency in Microsoft Office suite.

Responsibilities

  • Assess prospective clients for program eligibility through referrals and documentation; determine eligibility and conduct inspections as needed.
  • Provide direct support and guidance, connecting clients to housing, employment training, benefits, financial aid and counseling.
  • Maintain accurate and timely documentation of client interactions and outcomes per CCAC policies and regulatory requirements.

Role Overview

The Case Manager - Housing Stability determines client eligibility, conducts assessments and housing inspections, develops service plans, connects clients to resources, maintains accurate documentation, advocates for clients, collaborates with partners, and supports clients in achieving stability and self-sufficiency.

Job Responsibilities
  • Assess prospective clients for program eligibility through receipt of external referrals. Responsible for collecting and reviewing all documentation to determine program eligibility. Responsible for apartment inspections to ensure it is safe and meets habitability standards. Conduct comprehensive assessments to determine client needs and service eligibility. Develop, implement, and monitor individualized service plans in collaboration with clients.
  • Provide direct support and guidance to clients, offering emotional support and practical resources. Facilitate access to services, including housing assistance, employment training, mainstream benefits, financial aid and counseling.
  • Maintain accurate and timely documentation of client interactions, progress, and outcomes in compliance with CCAC policies and external requirements of regulatory bodies. Advocate for clients within the community and assist in navigating social services and resources.
  • Collaborate with multidisciplinary teams to coordinate services and ensure comprehensive care. Engage in outreach efforts to raise awareness of available programs and services.
  • Engage in ongoing learning, training, and skill development, seeking and applying feedback to continuously improve performance and contribute to team and organizational success.
Essential Functions
  • Kneel and move from sitting, bending, kneeling, or standing multiple times a day
  • Climb up and down up to 3 flights of stairs at a time
  • Lift, push, and pull up to 25 lbs

Education and Experience Requirements
Minimum
  • Bachelor's Degree in Arts/Sciences (BA/BS) 1-3 years relevant experience
  • Excellent written and verbal communication skills
  • Strong interpersonal skills
  • Ability to work effectively both independently and as part of a team
  • Proficiency in Microsoft Office suite
Preferred
  • Master's Degree (MA/MS) 3-5 years relevant experience
  • Prior case management experience
  • Spanish Profiecency (written and spoken)
Certifications and Licensure
  • Preferred LSW or LCSW

$25.00 - $28.00 / hour
