Job summary

Paycom - ATS is seeking a dedicated case manager to provide structured, long-term case management for residents in treatment programs. You will work with nurses on medication administration, maintain medication files, and coordinate counseling resources to support holistic client progress.

Responsibilities include regular client meetings, accurate records, and timely communication with nursing staff about admissions, changes in medications, and referrals.

Qualifications

• Bachelor’s degree or greater in Psychology, Social Work, Behavioral Science, or a related field.
• Three years progressively responsible experience providing direct case management services, preferably in a substance abuse treatment environment.

Responsibilities

• Meets regularly with clients to discuss and evaluate their progress and documents outcomes.
• Outsources or coordinates additional counseling resources to meet holistic client needs.
• Updates nursing staff on admissions, medication changes, and schedules. Maintains client medication files.

Schedule/Hours: Monday through Friday, 40 hours per weekThis position is responsible for:Provides comprehensive, long-term, structured, complex case management services for an assigned caseload of clients participating in residential treatment. Assists MAS nurse in medication administration, proper documentation, and reports medication administration errors. Creates and maintains client medication files. Evaluates client's progress by conducting mentoring and case management sessions with client and/or staff.Key Responsibilities:Meets regularly with clients to discuss and evaluate their progress. Prepares accurate and up-to-date records documenting the sameOutsources clients to additional counseling resources if needed. Maintains awareness of progress with external case management professionals, working as a team to meet the holistic needs of the individual.Updates nurse on a weekly basis of all new admissions into the DWL/NDP programs. Keeps nurse informed of any hospital admissions/discharges, changes in medication. Assists clients with medication refills and medical appointmentsCoordinates and/or facilitates life management, addiction related, behavior modification, etc. classes to assist clientsPhysical Requirements and Working Conditions:Work may involve driving a vehicle to off-site meetings where there may be physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like.
