Job summary

Paycom is hiring a dedicated case manager to support Veterans Outreach Program clients in Louisville, KY. You will conduct interviews, develop individualized service plans, and advocate for access to housing and community resources.

The role involves documentation in HMIS and coordinating housing transitions. The position requires a Bachelor's degree in a related field and at least three years of direct case management experience, with driving as part of duties.

Qualifications

• Bachelor's degree in Social Work, Behavioral Science, or a related field.
• Three years progressively responsible experience providing direct case management social services.
• Experience developing individual, comprehensive, long-term action plans for recovery using a wide variety of resources.

Responsibilities

• Interviews and provides comprehensive, long-term case management for an assigned caseload.
• Assists clients in locating/securing affordable housing and transitions into new housing.
• Prepares and maintains case records and HMIS entries.
• Occasional driving and field visits as part of service delivery.

Full Time: Mon - Fri, 8:30am - 4:30pm$19.02 hourlyInterviews, accepts, and provides comprehensive, long-term, structured, complex case management services for an assigned caseload of clients participating in an established life management program; understands the uniqueness of the client’s history to determine the most effective program plans; develops comprehensive program plan/goals and evaluates client's progress by conducting mentoring and counseling sessions with client and staff; serves as advocate for client to acquire services that will enable them to cope with their environment functionally; ensures constant compliance with funding requirements. Works with clients in the Veterans Outreach Program.Key responsibilities:Interviews, accepts, and provides comprehensive, long-term, structured, complex case management services for an assigned caseload of clients participating in an established life management program; understands the uniqueness of the client’s history to determine the most effective program plans.Assists clients in locating/securing affordable housing; works closely with clients to transition into the new housing; conducts home visitations when appropriate; ensures that client's living quarters are properly furnished and maintained.Prepares and maintains case records and logs on all assigned clients; ensures the accuracy and completeness of the same; enters pertinent information into the established Homeless Management Information System (HMIS).Physical Requirements and Working Conditions:Work requires the ability to lift and carry and/or push/pull light (less than 25 lbs.) materials, supplies, and equipment. Light mobility: duties are usually performed by a combination of standing, walking, and primarily sitting.Work is usually performed in a normal office environment where there is little or no physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like.
