Job summary

The Salvation Army in Nashville is seeking a dedicated Case Manager to provide long-term, structured services to clients in a life management program. You will conduct interviews, assess needs, and develop individualized service plans to support housing stability and independence.

Responsibilities include advocating for access to services, coordinating resources, documenting progress in HMIS, and ensuring compliance with funding requirements.

Qualifications

  • Bachelor's degree in Social Work, Behavioral Science, or related field is required.
  • Three years of progressively responsible direct case management experience are required.
  • Experience serving homeless or DV-affected clients is preferred.

Responsibilities

  • Interviews and assesses clients to develop long-term, structured case plans.
  • Provides comprehensive case management and mentoring to an assigned caseload.
  • Advocates for clients to access housing, services, and funding.
  • Ensures compliance with funding requirements and HMIS documentation.

Schedule: Mon-Fri 8:30 AM-5 PMThis position is responsible for:Interviews, accepts, and provides comprehensive, long-term, structured, complex, case management services for an assigned caseload of clients participating in an established life management program; understands the uniqueness of the client’s history in order to determine most effective program plans; develops comprehensive program plan/goals and evaluates client's progress by conducting mentoring and counseling sessions with client and/or staff; serves as advocate for client in order to acquire services that will enable them to functionally cope with their environment; ensures constant compliance with funding requirements. Domestic violence experience desired.Key Responsibilities:This job description should not be interpreted as all-inclusive. The incumbent may be requested to perform job-related responsibilities and tasks other than those stated in this job description.Case management is an essential function of this position and requires professional knowledge and experience in assessing client needs, developing individualized service plans, establishing measurable goals, monitoring and documenting client progress, coordinating services, maintaining accurate case records and HMIS documentation, facilitating supportive programming, and developing appropriate discharge and transition plans. The position requires the ability to evaluate complex client circumstances, identify barriers to stability and self-sufficiency, coordinate multiple internal and external resources, make appropriate referrals, monitor outcomes, and modify service interventions as necessary. These responsibilities require demonstrated knowledge and application of social service case management principles and the ability to work effectively with individuals experiencing complex social, behavioral, housing, and economic challenges.Given the population served, experience providing case management services to individuals impacted by domestic violence is a directly relevant and valuable qualification for this position. Domestic violence cases may involve safety risks, trauma, coercive control, housing instability, financial dependence, mental health concerns, and other circumstances requiring specialized assessment and service coordination. The individual in this role must be able to apply trauma-informed and safety-focused practices, maintain appropriate confidentiality, develop individualized goals without increasing client risk, and coordinate specialized community resources. Requiring or strongly preferring demonstrated domestic violence case management experience is therefore reasonably related to the essential functions of the position and supports the organization's responsibility to provide competent, appropriate, and effective services to a vulnerable client population.Working Conditions:Work is performed in a shelter environment where there may be exposure to physical health risks and unsanitary conditions.
