Main Street Direct is seeking a Case Manager responsible for managing a caseload of patients in a residential treatment setting. This role involves coordinating care and ensuring the delivery of comprehensive, patient-centered care while contributing to program effectiveness.
The ideal candidate will have a bachelor’s degree in social work or human services, or CASAC certification, and strong skills in crisis intervention and patient documentation.
Hours: Monday – Friday 8:00 AM – 4:30 PM.
Rotating Bi-Weekly evening shift (one day) Monday – Thursday 1:15 PM – 10:00 PM.
Rotating weekend shift every 6 weeks Saturday – Sunday 9:30 AM – 8:00 PM.
The Case Manager is responsible for managing an assigned caseload of patients within a residential substance use treatment setting, while also providing active support within the therapeutic milieu. This role involves coordinating care in collaboration with counseling staff and interdisciplinary team members to ensure continuity and quality of services. The Case Manager supports the delivery of comprehensive, patient-centered, wraparound care, contributing to the structure and effectiveness of daily program operations and activities.

Required: Possess a bachelor’s degree in social work, human services, or another closely related field, or possess a CASAC (Credentialed Alcoholism and Substance Abuse Counselor) certification, or be CASAC‑eligible with relative clinical experience in the substance use and/or mental health population.
Required: Must maintain a valid driver’s license and an acceptable driving record.
