Thrivenny in the Town of Evans, New York is seeking a Case Manager responsible for managing a caseload of patients in a residential substance use treatment setting. The role focuses on coordinating care and supporting patient needs while collaborating with a multidisciplinary team.
The ideal candidate will have a bachelor's degree related to social work or human services, and must possess a valid driver’s license. Responsibilities include overseeing patient activities, documentation, and promoting patient engagement.
Hours: Monday- Friday 8:00- 4:30 p.m.
Rotating Bi-Weekly evening shift (one day) Mon - Thurs. 1:15pm - 10:00pm
Rotating weekend shift every 6 weeks Sat-Sun 9:30am - 8:00pm
The Case Manager is responsible for managing an assigned caseload of patients within a residential substance use treatment setting, while also providing active support within the therapeutic milieu. This role involves coordinating care in collaboration with counseling staff and interdisciplinary team members to ensure continuity and quality of services. The Case Manager supports the delivery of comprehensive, patient-centered, wraparound care, contributing to the structure and effectiveness of daily program operations and activities.

Required: Possess a bachelor's degree in social work, human services, or other closely related field or possess a CASAC (Credentialed Alcoholism and Substance Abuse Counselor) certification or be CASAC eligible with relative clinical experience in the substance use and/or mental health population.
Required: Must maintain valid driver’s license and acceptable driving record.
