Qualificações

• Bachelor's degree in behavioral health, social services, or related field.
• 2+ years in behavioral health, social/human services.
• 2+ years of MS Office suite experience (Outlook, Word, Excel, SharePoint).
• 2+ years in children's mental health or related public sector field.
• 2+ years in family systems, case management, or related areas.
• Willing to travel up to 50% and possibly to New Albany office.
• Reliable transportation; mileage reimbursed.
• Flexible hours beyond 8-5 as needed.

Responsabilidades

• Evaluate referred members and plan care pathways.
• Coordinate and implement care plan activities and monitor progress.
• Collaborate with care teams to overcome barriers and ensure outcomes.
• Identify quality issues and escalate through approved channels.
• Serve as a single point of contact for members and assist with gaps in care.
• Travel within the assigned region to member homes as required.
