Qualifikationen

• Three years of hospital-based Case Management experience, including Utilization Review and Discharge Planning.
• Active license if licensure is required for the discipline within the hospital's state.
• For Nursing, must possess BSN with RN licensure.
• For other eligible health care professionals, minimum of a bachelor's degree; graduate degree preferred.

Aufgaben

• Oversee the day-to-day operations and human resource management of the Case Management department.
• Oversee the interdisciplinary plan of care and discharge planning to ensure effectiveness and appropriateness of services with a focus on census management and patient outcomes.
• Act as a patient and family advocate, ensuring services meet patient and family needs.
• Provide guidance and training to Case Managers and staff, including caseload management, regulations, policies, procedures, and objectives; review operations for quality.
• Build relationships with insurance companies, self-insured employers, case management firms, and other healthcare networks.
• Lead the Case Management leadership team as a key member of the executive leadership.

Ausbildung

BSN with RN licensure

Bachelor's degree; graduate degree preferred

CCM® or ACMTM certification (preferred)
