Job summary

Encompass Health in Abilene, TX is seeking a Case Management Director to lead the Case Management department, oversee the care plan, discharge planning, and census management to improve patient outcomes. You will mentor case managers and ensure compliance with regulations in a patient-centered environment.

You will build partnerships with insurers and healthcare networks while advancing a compassionate leadership culture across our organization.

Qualifications

• Current CCM® or ACMTM certification preferred.
• Independently complete assessments within a discipline's scope.
• Active license if licensure is required for the discipline.
• For Nursing, BSN with RN licensure.
• For other health care professionals, bachelor’s degree; graduate degree preferred.
• Three years hospital-based Case Management experience with utilization review and discharge planning.
• May require weekday/weekend/evening/night shifts.
• May work on religious and/or legal holidays as scheduled.

Responsibilities

• Oversee day-to-day operations and HR management of Case Management.
• Oversee interdisciplinary care plans and discharge planning with census management and outcomes focus.
• Act as patient/family advocate to ensure services meet needs.
• Provide guidance and training on caseload management, regulations, policies, and procedures.
• Review operations to maintain high quality aligned with standards.
• Build relationships with insurers, self-insured employers, and healthcare networks.
• Celebrate staff accomplishments along the way.
