Job summary

Encompass Health in Toledo, OH is seeking a Director of Case Management to lead the department, oversee the interdisciplinary plan of care, and drive high-quality patient outcomes through effective discharge planning and census management.

The ideal candidate will have CCM or ACMTM certification, hospital case management experience, and a BSN with RN licensure for nursing tracks; graduate degree preferred. Expect leadership responsibilities and collaboration with insurers and care networks.

Qualifications

• Current CCM® or ACMTM certification preferred.
• Qualified to independently complete an assessment within the scope of practice.
• Active license if required for the discipline in the state.
• For Nursing, BSN with RN licensure.
• Minimum of a bachelor's degree; graduate degree preferred.
• Three years of hospital-based Case Management experience, including Utilization Review and Discharge Planning.
• May be required to work weekdays and/or weekends, evenings and/or night shifts.
• May be required to work on religious and/or legal holidays on scheduled days/shifts.

Responsibilities

• Oversee day-to-day operations of the Case Management department.
• Oversee the interdisciplinary plan of care and discharge planning to ensure effectiveness and outcomes.
• Advocate for patients and families, ensuring services meet needs.
• Guide and train Case Managers; interpret regulations and procedures.
• Build relationships with insurers and healthcare networks.
• Celebrate employee achievements.
