Job summary

Encompass Health is seeking a Case Management Director in Fredericksburg, VA to lead a team and deliver exceptional patient care within our local community. This role supervises daily operations, care coordination, and discharge planning while guiding staff to achieve optimal outcomes and regulatory compliance.

The ideal candidate has CCM certification (preferred), an active license if required, and a minimum of three years hospital-based Case Management experience.

Qualifications

• CCM certification preferred
• Active license if licensure required for the discipline
• Bachelor's degree for nursing (BSN) with RN licensure; other disciplines require at least a bachelor
• Three years hospital-based Case Management experience, including Utilization Review and Discharge Planning

Responsibilities

• Oversee day-to-day operations and human resource management of the Case Management department
• Oversee the interdisciplinary plan of care and discharge planning to ensure effectiveness and quality
• Act as patient and family advocate ensuring services meet needs
• Provide guidance and training to Case Managers on caseload management and regulations
• Build relationships with insurance companies, self-insured employers, and healthcare networks
• Lead continuous improvement and celebrate staff achievements
