• Current CCM® or ACMTM certification preferred.
• Active license if required for discipline.
• Three years of hospital-based Case Management experience required.

Responsibilities

• Oversee day-to-day operations and human resource management.
• Ensure effectiveness of interdisciplinary plan of care.
• Act as patient and family advocate.
