Encompass Health Corporation is seeking a Case Management Director to lead the Case Management department and guide the interdisciplinary care team in a post-acute setting. You will oversee daily operations, planning, and discharge processes while advocating for patients and families.

The role requires CCM certification or equivalent, a nursing BSN with RN licensure if applicable, and at least three years of hospital-based Case Management experience. Weekends and holidays may be required.

Formación

• Current CCM® or ACMTM certification is preferred.
• Qualified to independently complete assessments within the scope of practice.
• Active license if licensure is required for discipline.
• For Nursing, BSN with RN licensure is required.
• Other eligible health care professionals should have a bachelor’s degree; graduate degree preferred.
• Three years of hospital-based Case Management experience including Utilization Review and Discharge Planning.
• May require weekdays/weekends, evenings, and/or night shifts.
• May require holidays on scheduled days/shifts.

Responsabilidades

• Oversee day-to-day operations and HR management of the Case Management department.
• Oversee the interdisciplinary plan of care and discharge planning to ensure effectiveness and census management.
• Advocate for patients and families to meet needs.
• Provide guidance and training to Case Managers and staff on caseload management and regulations.
• Build relationships with insurance companies, self-insured employers, case management firms and other networks.
• Celebrate accomplishments of employees while leading the team.
